As we near the end of quad one, we wanted to give families the opportunity to change their learning format if they choose for quad two. Please only complete this survey if:
- Your child is learning virtually and you would like to change to in person.
- Your child is learning in person and you would like to change to the virtual format.
If your child is continuing with their current learning format for quad 2, please do not complete this survey.
